The Source Intelligence program is extremely flexible in how we can accept data regarding conflict minerals. Below you will find a number of downloadable templates for providing the necessary information. If you wish to provide the information in another format please contact us and we will do everything we can to make the process as easy as possible for you. All of our tools are also designed to help you compling with other customer requests for the same information. By registering you have already taken the first step in meeting all of your customer's conflict minerals requests. Conflict Minerals Reporting Template 

The Electronic Industry Citizenship Coalition & the Global e-Sustainability Initiative have produced an Excel based Reporting Template that helps companies collect information related to conflict minerals. Revision two of the template allows for more detail reporting on the product level and contains a list of known smelters around the world. These templates are the most common way to share information but may require extra effort to gather the information from your suppliers about the origin of the conflict minerals used in your products:

If you need to have your suppliers, or their suppliers, complete the online version of the EICC-GeSI Due Diligence questionnaire you may download and complete this template with their contact information. Using the Source Intelligence platform you will be able to track the progress of the survey to understand when your suppliers have provided the requested information. The information regarding your suppliers will not be shared with your customers but will be securely stored within Source Intelligence.

Product Disclosure

US based companies are required to report the result of their inquires on a product basis. The document below will allow you to disclose which products that you supply are covered by your completion of the online EICC-GeSI template. As with information about your suppliers none of this information will be shared downstream but will only be used to aggregate the information to ensure accurate reporting.

Additional Smelters

The online version of the EICC-GeSI template is designed to reduce your reporting burden, but sometimes more information on suppliers needs to be entered. If you are aware of more mines or smelters in your supply chain than the online template allowed you to enter please download and return the document below with all the information you have been able to collect.
